CMNC Update Personal Information Form

Select one of the following links to join or renew: Non-member Mailing List, New Member, Renew Membership (or the Workshop forms when available). Submitting any of these forms will update or enter your information.

Use the form below to update only (when you don't need to join or renew) -- just enter your name, current/new email address, and any information that has changed, check carefully for typos, then click the Submit button at the end.

If you want to indicate you will not be renewing your annual membership, or if you want to be removed from the Database Mailing List, instead of simply updating, then please change UPDATE to one of the other choices

Non-members, take your best guess.
