If possible, instead please use the web form

  • Register if your're new to CMNC
  • fill out the web form online and click the Submit button - do not print it

If you need to use the following 2 page printable pdf form, a CMNC member will have to enter your written information into the web form, and there is also increased chance for error.

Forms and addresses change frequently:  Do not try to use a form you've previously downloaded.

Printable pdf (you may print double-sided or single-sided).

Please send an email to mail @ cmnc.org or call 510-828-4148 or include a comment with the printed form to explain why you were unable to use the online form. You will help us to understand problems and perhaps find solutions.
