Evaluation Form for COM '08 Workshop

Almost all-in-one: when you apply for a workshop, the workshop application (available for several weeks before each workshop) also contains a membership portion, so you can join or renew your membership at the same time, without submitting a separate membership form. Whether member or not, you are automatically placed on the mailing list and you can request an invitation to join CMNClist. Submitting the application also updates the information you've entered.

Mailing List (Non-Members)
   For non-members -- add your contact information to our database so we send you workshop announcements. (Members and workshop participants are automatically added to our mailing list, so they don't need to submit this form).

New CMNC Membership
   Become a member!  First-time (ever) membership is now $15. Benefits include annual directory of members, reduced workshop fees, newsletter, workshop announcements. You are added to our mailing list automatically when you use this form.

Renew CMNC Membership
   Renew your annual membership, which runs July 1 to June 30.

Make a Membership Payment to CMNC
   A payment page will appear after you submit a membership form.  If you were not able to complete payment at that time, you may use this convenient(!) page

Update Your Information (email, phone numbers, address, etc)  
   A rolling stone gathers no workshop announcements from us.  Please keep your contact information up-to-date (if you are a member or are on our mailing list).  Thanks.
   You can also tell us you do not plan to renew, or you want your contact information removed from our database.

If you want your name changed in our records, please send an email to mail@cmnc.org detailing the old and new names.

How to Join and Use CMNClist 
   CMNClist is a moderated email service which allows you to send and receive emailed messages of interest to fellow chamber musicians (such as upcoming concerts or player seeking other players). All of the above forms (except Evaluation) allow you to indicate you would like to join CMNClist.

    We prefer that you use the web forms when possible. Otherwise, download and mail the printable forms.
